PERSONAL RESPONSIBILITY AND RELEASE OF HEALTH CARE RELATED CLAIMS:

The student acknowledges that the student takes full responsibility for the student’s life and well-being and all decisions made during and after this class.  The client expressly assumes the risks of the class, including the risks of trying new activities, energy healing, and universal connections and the risks inherent in making lifestyle changes.  The student releases the practitioners from any and all liability, damages, causes of action, allegations, suits, sums of money, claims and demands whatsoever, in law or equity, which the student ever had, now has or will have in the future against the practitioners, arising from the student’s past or future participation in, or otherwise with respect to, the class.

I understand that the practitioners do not diagnose conditions nor do they prescribe or perform medical treatment, prescribe substances, nor interfere with the treatment of a licensed medical professional. I understand that these classes do not take the place of medical care. It is recommended that I see a licensed physician or licensed health care professional for any physical or psychological ailment I may have. I understand that these classes can complement any medical or psychological care I may be receiving.
